Fill out this form to register for a Parent/Child Dedication service at Wawasee Bible.

The following forms are currently only available as a download in PDF format. You will need to download them, print them, fill them out, and return them to the church office.

Facility Usage Request

Download, print, and complete the Facility Usage Request form to request usage of the facility of Wawasee Community Bible Church. Completed requests should be returned to the church office no later than 30 days prior to your event date for approval. We authorize use of the facility only for not-for-profit events and organizations. If approved, any applicable fees will be due 15 days prior to your event or your event will be at risk of cancellation.

We will work to make a decision on your request within 30 business days. Please recognize that in some instances this will not be possible, however, and we will respond as soon as possible. Facility requests made prior to 6 months of the event date can be subject to modification or cancelation if your event comes into conflict with the ministry needs of Wawasee Community Bible Church.

• All rental fees below are waived for members and regular attendees of Wawasee Bible who utilize the facility for personal/family events. Rental fees still apply if you are reserving the facility for an outside organization. If you have committed to fully cleaning your facility area (see form), cleaning fees are waived as well.
• Weddings follow a separate fee schedule, see your Wawasee Community Bible Church Wedding Application and mark total fees as “Wedding” below. Members and regular attendees of Wawasee Bible are still required to pay all fees listed in the Wawasee Community Bible Church Wedding Application.
